Virginia Beach All-on-4 implants, a fixed arch on four posts

All-on-4 carries a whole jaw of teeth on four implants. The back two are tilted forward to catch denser bone nearer the front, which is often what lets an upper arch go ahead without a sinus lift.

The angle

Virginia Beach All-on-4 tilts the back posts to find bone there

All-on-4 implants exist because bone at the back of the upper jaw is often thin, the sinus sitting low and the ridge has shrunk. Placing a post straight up there frequently means lifting the sinus first. Angling the rear posts forward instead lets them anchor in the denser bone in front of the sinus, avoiding a stage that would otherwise add months.

The All-on-4 angle also spreads the four posts further apart than four upright ones would sit, which gives the bridge a wider base. Where the jaw has bone to take more, All-on-6 Implants distributes the load further still, and Full Mouth Implants covers doing both jaws.

How it works

All-on-4 implants, from failing teeth to a fixed arch

  1. Scan and plan the angles

    A CBCT scan maps bone across the jaw and the sinus position. The tilt of each rear post is worked out here, not in surgery.

  2. Surgery day

    Remaining failing teeth come out, four posts go in, and a temporary bridge is fitted the same day where the posts are stable enough.

  3. Soft months

    Bone fuses to the posts over several months while you eat softer food. Reviews along the way rather than only at the end.

  4. The final arch

    The permanent bridge is made to the shape and shade agreed with you, fitted, and checked against the opposing bite.

Honest limits

Four posts is lean by design

All-on-4 is enough for most jaws and is chosen precisely because it needs less bone. That leanness is also its limit: there is no spare post if one ever fails, so the arch has to come off while the site is treated. Six gives more margin, which is the trade set out on All-on-6 Implants.

Usually yes. A temporary bridge goes on the same day where the four posts are stable enough to carry it. If the bone is soft or an infection has to settle, you may wear a temporary denture for a few weeks instead. We tell you which is likely before the day rather than after.
Yes, because a fixed bridge spreads its load across all four rather than loading each tooth separately. The protocol has decades of use behind it. What matters is that the four are placed where the bone can actually hold them, which is what the planning is for.
Often, and that is much of the appeal. Angling the rear posts forward reaches bone in front of the sinus. It is not certain either way; where the ridge has resorbed severely a lift may still be needed. The scan answers it before you commit.
With only four, losing one usually means the bridge comes off while the site is treated and a replacement post placed. It is uncommon but it is the honest downside of the lean approach, and it is the main reason some cases are planned with six instead.
Brushing as normal plus cleaning underneath where the bridge meets the gum, usually with a water flosser or interdental brushes. That junction is where problems start, so the routine matters more than with natural teeth. We show you at the fit appointment.
